Online form for changing authentication method (phone number)

TheNational Health Service of Ukraine (NHSU)recommends using this online form:

• if, when renewing a declaration for choosing a primary care physician, the confirmation SMS is sent to an unknown phone number;
• if, when issuing a prescription under the “Affordable Medicines” program, the confirmation SMS is sent to an unknown phone number;
• in case of loss or change of the previous phone number;
• in case of correcting an incorrectly entered phone number;
• for changing the authentication method without using a phone number, which requires uploading documents to verify the patient’s information.
